Missed your GLP‑1 dose? What the label says, drug by drug
It happens to everyone: travel, a pharmacy delay, or you just plain forgot whether Thursday was shot day. The good news — every GLP-1 label has an explicit missed-dose rule, and most give you more slack than you'd guess. Here they are in one place.

Zepbound & Mounjaro (tirzepatide)
The label gives you a 4-day (96-hour) window after your scheduled time. Miss Wednesday's shot and remember on Saturday? Take it. Remember on Monday? Skip it and take your next dose on Wednesday as usual. The hard constraint: at least 3 days (72 hours) between any two doses.
Wegovy (semaglutide 2.4 mg)
Wegovy's rule is anchored to your next dose, not the missed one: if the next scheduled shot is more than 2 days (48 hours) away, take the missed dose as soon as you remember. Less than 2 days away? Skip it and resume your normal day. One extra Wegovy-specific rule: miss more than 2 consecutive weeks and the label says to talk to your prescriber before restarting — you may need to step back down and re-escalate to avoid a rough week of nausea.
Ozempic (semaglutide)
Simplest window of the weekly shots: take within 5 days of the missed dose, otherwise skip. Minimum 48 hours between doses.
Trulicity (dulaglutide)
Take the missed dose as soon as possible if there are at least 3 days (72 hours) until your next scheduled dose. Closer than that, skip it.
Saxenda & Victoza (liraglutide — daily shots)
Daily meds work differently: there's no making up a missed day. Just take your normal dose the next day — never a double dose. The important rule is the 3-day cliff: if you've missed more than 3 days in a row, the label says to restart at the lowest dose (0.6 mg for Saxenda) and re-escalate, because jumping straight back to your full dose invites serious nausea. That restart is a prescriber conversation, not a solo decision.
Rybelsus (oral semaglutide — daily pill)
Missed this morning's pill? Skip it entirely. No afternoon make-up — the pill only absorbs properly on a truly empty stomach. Tomorrow morning: empty stomach, no more than 4 oz of plain water, wait 30 minutes before food, drink or other medications.
Changing your shot day on purpose
Travel, a new work schedule, or you just hate Sunday shots — all fine. The only rule is the minimum gap between doses:
- Semaglutide (Wegovy, Ozempic): at least 2 days (48 h) between doses
- Tirzepatide (Zepbound, Mounjaro): at least 3 days (72 h) between doses
- Dulaglutide (Trulicity): at least 3 days (72 h) between doses
Pick the new day, make sure the gap from your last shot clears the minimum, then stay weekly from there.
Why doses get missed in the first place
In the community, the big three are: couldn't remember if it was taken (the "did I already shoot?" panic), pharmacy shortages breaking the routine, and travel. All three have the same fix — a written record. Whether that's a notebook, a spreadsheet, or an app, know your last dose date and site before the question ever comes up.
